About Western Linkages

Western Linkages is a Home and Community Care (HACC*) funded project operating in Western Metro Adelaide.  The role of the project is to:

  • Progress the goals and directions of the HACC program
  • Focus on improved service provision for older and frail people, younger people with disabilities and their carers.

This is achieved through the project by:

  • Bringing both HACC and non-HACC funded service providers together for sharing knowledge and experience
  • Working together for the reduction of undesirable duplication in services, identifying service gaps and improving/reforming current practices.

Western Linkages facilitates a range of collaborative strategies, including:

  • Western Linkages Steering Committee – a diverse group of service providers and consumer representation, that meets monthly to guide the project and provide a platform for regional issues
  • Regional Forums – one-off regional events, usually with a focus or theme
  • Workgroups – groups of service providers that meet regularly to target and discuss common issues.

Participation is open to any service provider with an interest in the western metro region.

*HACC eligible clients are people who are frail aged, younger people with a disability and their carers, who live at home and whose capacity for independent living is at risk or who are at risk of premature or inappropriate admission to longterm residential care.
