Mind the Gap

Mind the Gap is an online survey collecting information (anonymously) on situations where agencies have been unable to assist a person seeking social support. As of July we have collected data on 109 clients across the West and North metro regions. Any service provider can participate by clicking on the survey link . Regular reports are provided through the Social Isolation Workgroup. Northern services, click the Mind the Gap link at the Northern Collaborative Project www.salisbury.sa.gov.au

The Western Linkages Project

Facilitating collaboration and reform across community service providers.

Facilitating Care

Western Linkages is a collaborative project that was established in 2000 by a group of community service providers in Western Adelaide. The project’s role is to facilitate reform, networking and collaboration across community service providers that work with HACC eligible clients* in the Western Adelaide region.

Participation is open to any service provider with an interest in the western metro region.

*HACC eligible clients are people who are frail aged, younger people with a disability and their carers, who live at home and whose capacity for independent living is at risk or who are at risk of premature or inappropriate admission to longterm residential care.
